Our network of partners
Olympic Connect is a Community Care Hub of Washington. A community-driven network of partners on the Kitsap and Olympic Peninsulas, we support care coordination to local resources like health care, food, childcare, housing, and more – to help everyone in our region get the support and care they need to thrive.
Olympic Connect brings together partners from many different backgrounds, sectors, communities, and Tribes. Building bridges between and among the community and clinical workforce to create a more person-centered approach to health.
Everyone has a role to play in this important work to create a healthier, more equitable three-county region.
Care Coordination Partners:
Care Coordination Partners are the backbone of Olympic Connect. These partners provide community-based care coordination services to support community members in reaching their life goals. They employ members of the community-based workforce. These trusted helpers provide one-on-one, tailored, and culturally relevant support to community members for services like housing, job training, food assistance, health care, childcare and more. Each partner has a unique perspective to lend.
Resource and Service Partners:
Olympic Connect works closely with partners throughout the three-county region to ensure there are sufficient resources to meet the needs of community members. We maintain a robust resource directory so anyone can learn more about how to connect with services like housing, food, job training and job connections, transportation services, and more. Olympic Connects provides funding to some partners to expand and enhance access to social care.
Advisory Group:
The Olympic Connect Advisory Group provides recommendations to the Olympic Community of Health Board of Directors and staff on the successful implementation of Olympic Connect. The advisory group meets quarterly, and participation is open to anyone who lives, works, or plays in the Olympic region. Contributing to the advisory group is one of many ways to partner with Olympic Connect and meaningful participation from all partners and community members interested in supporting this work is welcome.
Governance
Olympic Connect is a service of Olympic Community of Health. The Olympic Community of Health Board of Directors provides governance and oversight to Olympic Connect. This is a dynamic and collaborative group of health leaders from throughout the three-county region including Tribes, Health Care Providers, Community-Based Organizations, and more. Board meetings are open to the public.
Partner with us
No one can do this work alone. We invite every organization and Tribe in the Olympic region to join Olympic Connect. There are many ways to partner with us from being a connector for community members by referring to Olympic Connect to becoming a local champion participating in the advisory group. Refer patients and clients
Anyone can access Olympic Connect, regardless of insurance or Medicaid status. Accessing Olympic Connect is free, though some services may have a cost.
